Individual or close family member with any of the following:
Known mutation (pathogenic variant) in a hereditary cancer syndrome gene
Cancer diagnosed before age 50
Ovarian cancer, pancreatic cancer or advanced prostate cancer
Metastatic breast cancer (patient only)
Multiple close family members on the same side of the family with the same or related cancers (ex. breast and ovarian; colon and uterine)
Two or more primary cancers diagnosed in the same individual
Rare cancers or tumors such as triple negative breast cancer, male breast cancer, medullary thyroid cancer, adrenocortical carcinoma, pheochromocytoma
10 or more colon polyps
Ashkenazi Jewish (Eastern European) ancestry
Variant of uncertain significance in a hereditary cancer risk gene not already evaluated by a genetics professional
Incomplete prior genetic testing- testing without multi-gene evaluation or with incomplete multi-gene testing (typically any testing prior to 2014, please ask NCGenetics to review prior report)
Please complete and fax the genetic counseling/testing order form along with a recent chart note and NCGenetics will contact the patient to schedule an appointment. The order form will be used to order genetic testing under your name as you so authorize. Please don't hesitate to call or email if you have any questions about a case.
